Am I a candidate for laser cataract surgery?

Not everyone is a candidate for laser cataract surgery, and we highly recommend a consultation for a personalized surgical plan. One size fits all vision approaches are a thing of the past. The factors that make someone a medically good or poor candidate for laser cataract surgery include the presence of and location of corneal scars, history of certain kinds of eye surgeries or trauma in the past, and the anatomy of the conjunctiva, or the white part of the eye, and its underlying structures. No one has the same fingerprint or eyes!

When should I get cataract surgery?

You know, with cataract surgery its a little different from having LASIK or having PRK. With LASIK and PRK, if I decide I want to wait, then, I can wear glasses and contacts and likely still see 20/20. With a cataract, by definition, somethings clouding my vision that glasses wont fix. So how long do you want to wait? Now, even so, cataract surgery is elective surgery. We always tell patients its nothing to be apprehensive about because it is such good surgery. And the safety record is so good.

Bmc: You Mention Getting Involved In Technologies Early On You And Stephen Brint Md Were The First Two Us Surgeons To Perform Lasik How Did You Get To Be Involved In That What Was It Like Performing That First Procedure

Slade: I was doing lamellar keratoplasty with a keratomemyopic keratomileusiswhich was sort of a precursor to LASIK. Steve Brint was one of the first ophthalmologists to get an excimer laser. We put the two together for something we called excimer laser myopic keratomileusis.

We would take about two-thirds of the cornea off, about 350 µm, with a manual keratome. Then we would take that piece of corneal tissue, walk it over to an excimer laser, modify it with the laser, then bring it back and sew it onto the patients eyeneedless to say, it was an involved procedure. The laser manufacturer at the time, Summit, started a trial called the Excimer Laser Myopic Keratomileusis Trial, and Steve invited me to be an investigator, so I got a Summit excimer laser.

That was a wild trial. We would take the patient into the OR, slice off that big hunk of cornea, and then literally put on coats and walk the tissue across the street to the building that housed the laser, go up the elevator, treat the tissue, come back down, go back into the OR, rescrub, and sew it back on. One time we got on the elevator to go back up to the OR, and there was the patients family. They knew that we were operating on their family member, but there we were, me and Steve, with all of them in the elevator together. It was awkward.

Slade: You know, Im not sure. We did have the first laser cataract surgery device in the United States and were the first to do the surgery. We had the first laser flapmaker in the United States, the IntraLase . We implanted the first Crystalens , and weve had few other firsts.

Honestly, this may sound like a copout, but I think a lot of it is just luckknowing the right people and being in the right place at the right time. My contribution might be that I understand how to work with early-stage companies and technologies. When companies come to you with an idea, you have to realize that youre sort of the test dummy, if you will. Its a huge responsibility because youre using it on your own patients. Theres no one to learn from because youre the first. You have to innovate as you go along and figure out the best techniques and how best to use it, whatever it may be.

When you work with early-stage technologies, you have to know how to be honest and frank with the company about what works and what doesnt. You also have to know that you talk to the company, you dont go out and shout to the world, This device doesnt work. Instead, tell the company, This is the problem. You must be part of the team.

Even more importantly it is also a serious deal for the study participants and potential patients for any new technology. I feel an obligation to those patients to do the best I can.

The Lasik Procedure Explained

During LASIK eye surgery, the goal is to remove corneal tissue using an excimer laser, gently reshaping the cornea. This changes the focusing power of your eye, allowing for sharp vision.

Before LASIK surgery, you will come in to our office in Houston for a consultation. The consultation will make sure you are a good candidate for LASIK surgery.

Before the LASIK procedure, your doctor in Houston will make the necessary pre-operative preparations. You are positioned under the laser microscope, with a lid speculum to hold your eyelids back, so you do not blink during the procedure. Next, our doctors use a microkeratome device to create a thin corneal flap. The flap is then lifted back, exposing the inner layers of the cornea. The next step is to use the excimer laser to reshape the cornea, using the specifications that were prepared before surgery. Lastly, the flap is smoothed back down and reseal itself, requiring no stitches.
